### **User Manual for New Server Provisioning Form**  
*University of Connecticut - IT Services*

### **1. Purpose**

This document aims to empower users to successfully submit server provisioning requests through the Jira Service Management form, leading to streamlined and accurate server builds.

**  Functionality:**

These manual details the process of completing the server provisioning request form in Jira Service Management. The form is designed with conditional logic to tailor the required information based on user selections, such as:

- Server type (web server or not)
- Standard or customized build requests
- Operating System/Zabbix Group
- Department/Organizational Unit
- Existing Business Service or not

This dynamic functionality ensures that users are only prompted for information relevant to their specific server provisioning needs.

**Active Directory Placement:** Upon successful provisioning, all servers will be automatically added to the `UConn\managed_server_program` organizational unit within Active Directory.

### **2. Accessing the Form**

**Only IT professionals with a Jira license can access the form.**

1. Open a web browser and navigate to the following link:

2. Log in with your UConn Email and NetID credentials if prompted.

### **3. Completing the Form**

Fill in the required fields with accurate information to avoid delays in processing your request. Below is an explanation of each section of the form:

**1). Raise this request on behalf of: **  You can submit a server build request on behalf of a faculty/staff member or another individual using this form. The named requestor will receive email notifications for ticket creation and status updates (approval/denial and build completion).

#### ** Information Section**

Before proceeding, review the standard server build specifications provided in the form:

- **Windows:** 8 GB Memory, 4 CPU Cores, 160 GB C Drive, 100 GB E Drive
- **RHEL:** 8 GB Memory, 4 CPU Cores, 160 GB Drive

If your RHEL build requires more than the default disk space, please specify the required amount in the "**Additional Comments**" field.

Requests exceeding these standard specs will require additional approval.

#### **2).  Server Standard Build Specs Meet Needs:**

Default answer is "Yes" (use recommended values). Select "No" to enter custom values for CPU, Memory, and Windows E Disk Space.

For RHEL server, leave the default response on “Windows E Disk Space”.

**3).** **Server Type Selection**

- **"Will this be a Web Server?"**
  - Click the dropdown and select:

**Yes** – The 'Type of Web Server' selection will appear—choose an option from the list below.

**No** – If the server is not a web server.

**Conditional Actions Based on Selection:**

- **If "Yes" is selected:**
  - The UConn Accessibility and Compliance Team will review the request upon form submission.
  - The Accessibility and Compliance Team will close the Jira ticket when deemed appropriate.
- **If "No" is selected:**

       The Jira ticket will be closed once the server build is complete.

**Fill server build related information** - ** **

**4). Additional Comments(optional):  **Please use this space to provide any specific instructions for the Virtualization & Storage team to ensure the server is built according to your requirements.

**5).  Server Name:**

     Enter a unique name for the server. The name must be no more than 15 characters.

**6). Operating System:**

Select the desired operating system for the server:

RHEL_9_64

WINDOWS_SERVER_2022

WINDOWS_SERVER_2025

The '**Zabbix Group**' field appears only when **RHEL_9_64** is selected. You can choose from the dropdown list to assign the server to the appropriate group, ensuring it is monitored by Zabbix, and alerts are sent to the correct email distribution list.

**7). Purpose of Server**

Enter a detailed description of the purpose of the requested server. 

**8). Service Level:**

     Select the service level for the server:

Production

Development

**9). VLAN:**

     Select the appropriate VLAN for the server. The default VLANs are:

      Windows: 220_SvrSys_Windows_MSB & Linux: 219_SvrSys_Linux_MSB

**10). Department:**

     Select the department that this server belongs to.

     When "ITS" is selected, the **Organization Unit** field appears. By clicking on the dropdown list, you will see a list of OUs under the ITS OU within the Active Directory structure of **UConn\Managed_Server_Program**.

**11). Business Service Name**

    This field requires you to select the business service that this server belongs to.

     To find your service, search by Org Unit (e.g., enter 'Server_Systems'). If your service isn't listed, select '**Other**' and enter the service name in the '**New Service Name**' field.

**12). NetID:**

Select your NetID from the dropdown. Only **UConn employees** are listed.

If you're submitting this request for another user, enter your NetID. The associated NetIDAdmin account will be added to the local administrators group.

To grant additional users administrator access, enter their NetIDAdmin accounts in the 'Other Server Admins' field.

**13). Business Service Owner:**

     Enter or select your NetID from the dropdown list. Only UConn employees are included.

**14). Other Server Admin's:**

      Enter the NetIDAdmins of any additional administrators who will have access to the server. Separate multiple NetIDAdmins with commas.

**15). Will this Server Contain PII?**

      Select "Yes" if the server will store any personally identifiable information (PII). Otherwise, select "No."

**16). Network Zone:**

      This field is used for Infoblox purposes. The default value is “its.uconn.edu”

**17). Storage Cluster:**

     Select the desired storage cluster for the server. The default cluster is "MSB-B Cluster - EVC."

**18). Zabbix Group**

     This field appears only when **RHEL_9_64** is selected. You can choose from the dropdown list to assign the server to the appropriate group, ensuring it is monitored by Zabbix, and alerts are sent to the correct email distribution list. The default group is "enterprise_systems."

** Send:**

Click the "Send" button to submit the request.

**Additional Notes:**

- **Ensure all required fields are completed correctly. Only ‘Additional Comments','Other Server Admins' and 'Zabbix Group’ are optional.**
- Provide as much detail as possible in the "Purpose of Server" field to help with the provisioning process.

### **4. After Submission**

Your request will be reviewed by the IT Services team.

If additional information is required, you may receive follow-up questions.

Once approved, the server provisioning process will commence, and you will be notified upon completion.

**Updating Your Submission**  
If you need to update the information in the form after your initial submission, follow these steps:

1. Click the three dots (…) as shown on the screen.
2. Select **‘Edit form’**, make the necessary changes, and click the **‘Submit’** button to resubmit.
