If you have an issue with your device, you can create a service request. These requests are designed to help you let Motus know about general questions, device issues, requesting additional features, and more. Submitting a request will send a ticket to Motus's Account Support team to review and resolve.

After selecting the **Replacement Orders** or **Review and/or Modify Existing Lines of Service** link on the landing page, you will see this page with your information listed at the top. For user security, information is blacked out.
