<span style="color: #172b4d">This article is for faculty, staff, and students who want to create a form, quiz, or survey using OneDrive for Business.</span>

<span style="color: #172b4d">With Microsoft Forms, anyone can create surveys, quizzes, and polls, invite others to respond to it using almost any web browser or mobile device, see real-time results as they are submitted, use built-in analytics to evaluate responses, and export results to Excel for additional analysis or grading. </span>

## <span style="color: #172b4d">Creating a Basic Form</span>

1. Sign in to OneDrive at [onedrive.live.com](http://onedrive.live.com).
2. Click **New **and choose **Forms for Excel** from the drop-down menu.
3. <span style="color: #1e1e1e">Choose a name for the form.</span>
4. <span style="color: #1e1e1e">Click </span><span style="color: #1e1e1e">**Add New**</span><span style="color: #1e1e1e"> and choose the type of question.</span>
  - <span style="color: #1e1e1e">For </span><span style="color: #1e1e1e">**Choice**</span><span style="color: #1e1e1e"> questions, add a question and options. If multiple selections should be allowed, enable </span><span style="color: #1e1e1e">**Multiple answers**</span><span style="color: #1e1e1e">. </span><span style="color: #1e1e1e">**Shuffle options**</span><span style="color: #1e1e1e"> and </span><span style="color: #1e1e1e">**drop-down**</span><span style="color: #1e1e1e"> can also be enabled by clicking the </span><span style="color: #1e1e1e">**...**</span><span style="color: #1e1e1e"> icon in the bottom right corner of the question.</span>
  - <span style="color: #1e1e1e">For </span><span style="color: #1e1e1e">**Text**</span><span style="color: #1e1e1e"> questions, add the question. If a long answer is desired, it can be enabled by selecting the </span><span style="color: #1e1e1e">**Long answer**</span><span style="color: #1e1e1e"> icon.</span>
  - <span style="color: #1e1e1e">For </span><span style="color: #1e1e1e">**Rating **</span><span style="color: #1e1e1e">questions, add the question. Then, choose the number of </span><span style="color: #1e1e1e">**Levels**</span><span style="color: #1e1e1e"> and the </span><span style="color: #1e1e1e">**Symbol**</span><span style="color: #1e1e1e"> to be used. The symbols can be labeled by clicking the </span><span style="color: #1e1e1e">**...**</span><span style="color: #1e1e1e"> icon in the bottom right corner of the question and then selecting </span><span style="color: #1e1e1e">**Label**</span><span style="color: #1e1e1e">.</span>
  - <span style="color: #1e1e1e">For </span><span style="color: #1e1e1e">**Date**</span><span style="color: #1e1e1e"> questions, add the question.</span>
  - <span style="color: #1e1e1e">For </span><span style="color: #1e1e1e">**Ranking **</span><span style="color: #1e1e1e">questions, add the question and options.</span>
  - <span style="color: #1e1e1e">For </span><span style="color: #1e1e1e">**Likert**</span><span style="color: #1e1e1e"> questions, add the question, statements, and options.</span>
  - <span style="color: #1e1e1e">For </span><span style="color: #1e1e1e">**File upload**</span><span style="color: #1e1e1e"> questions, add the question, file number limit, and single file size limit. To limit the types of files allowed, click the </span><span style="color: #1e1e1e">**...**</span><span style="color: #1e1e1e"> icon in the bottom right corner of the question, select </span><span style="color: #1e1e1e">**File type**</span><span style="color: #1e1e1e">, and check the files that should be allowed.</span>
  - <span style="color: #1e1e1e">For </span><span style="color: #1e1e1e">**Net promoter score**</span><span style="color: #1e1e1e"> questions, add the question and the scale labels.</span>

<span style="color: #1e1e1e">All questions can be made required by selecting the </span><span style="color: #1e1e1e">**Required **</span><span style="color: #1e1e1e">icon.</span>

## <span style="color: #1e1e1e">Creating Sections</span>

<span style="color: #1e1e1e">Sections break the form up into multiple pages. Instead of seeing all questions on one page, users will only see one section at a time. To navigate to the next section, all required questions must be answered and the user must click the </span><span style="color: #1e1e1e">**Next**</span><span style="color: #1e1e1e"> button. </span>

1. <span style="color: #1e1e1e">Click </span><span style="color: #1e1e1e">**Add New.**</span>
2. <span style="color: #1e1e1e">Select </span><span style="color: #1e1e1e">**Section.**</span>
3. <span style="color: #1e1e1e">Enter a section name, and if desired, a description.</span>

## <span style="color: #1e1e1e">Adding Branching</span>

<span style="color: #1e1e1e">With branching, a user's response to a question can dictate what happens next in the survey. On any question, branching can be added by:</span>

1. <span style="color: #172b4d">Select the </span><span style="color: #172b4d">**...**</span><span style="color: #172b4d"> icon in the bottom right corner of the question.</span>
2. <span style="color: #172b4d">Click </span><span style="color: #172b4d">**Add Branching**</span>
3. <span style="color: #172b4d">Next to an option or at the end of a question, choose the question or section the survey should go to next.</span>

## <span style="color: #172b4d">Adjusting Form Settings</span>

1. <span style="color: #172b4d">Click the </span><span style="color: #172b4d">**...**</span><span style="color: #172b4d"> icon in the top right corner of the screen.</span>
2. <span style="color: #172b4d">Select </span><span style="color: #172b4d">**Settings.**</span>

<span style="color: #172b4d">On this menu, you will have the ability to enable the following features:</span>

- <span style="color: #172b4d">Record name</span>
- <span style="color: #172b4d">One response per person</span>
- <span style="color: #172b4d">Accept responses</span>
- <span style="color: #172b4d">Start date</span>
- <span style="color: #172b4d">End date</span>
- <span style="color: #172b4d">Shuffle questions</span>
- Customize thank you message
- Send email receipt to respondents
- Get email notification of each response

## Changing the Form Theme

To change the form theme, click the **Theme** button on the top bar. There are many default themes as well as the ability to create a custom theme by clicking the **+** icon.

## Previewing the Form

Previewing the form allows the creator to view the form as someone filling it out. To preview the form, click the **Preview** button on the top bar.

## Share Form

To share the form with others, click the **Share** button on the top bar. There are four different sharing options:

1. A link that can be posted or shared.
2. A QR code that can be scanned.
3. Embedded HTML.
4. Email.

Additionally, the form can be shared as a template so others can create their own form or to add others as editors.
