Students, faculty, and staff can alter information, troubleshoot issues, or request voicemail service for their landline phones. 

### Phone Request Form

This form can be used to add, disconnect, change, or move a landline phone.

### KFS Account/ Subscriber Name Change Form

This form can be used to change the payment code applied to a landline phone or change the name of the subscriber who has the landline phone.

### Other Form

This form should be used for any circumstances regarding your landline phone that do not apply to the two forms previously explained.

### Disconnect Cellular Service Form

This form should be used by faculty or staff who wish to return a working university-issued cellular phone after it has been used or to return a cellular phone that is n<span style="color: #172b4d">on-working or old. </span>
