Students, staff, and faculty may report a damaged telephone jack in classrooms and conference rooms across campus.  ITS can make room visits to fix issues with these jacks.

## Filling Out a Data Jack Repair Request

You can submit a ticket to the ITS Technology Support Center through the [web portal](https://techsupport.uconn.edu/) or by sending an email to [techsupport@uconn.edu.](mailto:techsupport@uconn.edu.)

If possible, provide the following information in the ticket or email:

- First and Last Name
- Phone Number
- Email Address
- Building
- Room Number
- Description of the problem

## Filling Out a Student Telephone Jack Repair Request

If you are having problems with your dorm phone (no incoming or outgoing calls, voicemail problems, no ringing, etc.) fill out the [Student Telephone Trouble Ticket](https://student.phone.uconn.edu/student-telephone-trouble-ticket/)*.*

Provide the following information:

- Name
- Contact number (use alternate if your dorm phone is not working)
- Email
- Residence Hall or apartment building
- Room number
- Campus Phone number
- You will need to be present to allow a technician to enter the room and repair the jack. Under "Access Times & Trouble Information" list two different two-hour time blocks, Monday through Friday, 8:00 AM – 4:00 PM that you will be available for the technician's visit.
- Under "Trouble Information," check off the problem(s) that you are having with the telephone. Make additional comments in the appropriate section as necessary. Select **Submit** once done.
